  1. Mechanical failure? No, Computer failure. Modern ships have computer controlled engines and transmissions and when the computer crashes so does the ship.

  3. Cruise ship pilot made a good decision to wedge the ship between the dock in the boat pushing it away rather than crushing it against the dock making the situation much worse.
